We need all 4 sheets to have our "EDP Dental Plan" logo in the left corner of all 4 sheets. Only the "EDP will be blue and the semi-circle blue to fade. The "Dental Plan" under EDP in Blue should be black and bold. We would like all the terminology the same, just laid out in a more professional manner. Please also put in the bottom right corner of each sheet a Edp number. For example: Edp-101. So each sheet will have it's own number. On the EDP Dental Plan Fee Schedule, feel free to box some in, use lines, or any way you can but keep the codes in order. VERY IMPORTANT.
